All kits are dried blood spot. You can either collect your blood samples at home on your own or I can collect the samples in office.

You will include payment information with your test sample to pay Alletess directly.

Getting Your Labs Done

Contact Bio-Stream Lab. You can go to their patient service center in Citrus Heights to have your blood drawn for no additional charge or you can arrange for a mobile phlebotomist to come to your home for a price of up to $10. 

Contact Bio-Stream to make an appointment for either service. It is preferred that you have an appointment. You can also attend walk-in hours. They have two locations:

Payment

If you have a qualified insurance, you will be billed the Know It Now price. You will receive your bill in the mail after you get your labs done. If you pay your bill within 14 days of receiving it, you will receive 30% of the Know It Now price. Follow instructions on your bill. You will receive an Explanation of Benefits letter from your insurance company explaining what they did and did not cover. This is NOT a bill. You do NOT owe the difference.

If you are paying cash, you will need to pay for your labs before you get your blood drawn.  Call 877-425-1602, choose option 4. Say you are a "direct pay" patient. Give them the client ID 8635. Provide all test codes. They will then give you an auth code. Please write this on your lab requisition form in the Billing box.

Cyrex

Getting Your Labs Done

Cyrex arrays all require a blood sample. In office, I will provide you with a requisition form, lab kit, and handout. You will need to follow the instructions on the handout to get your blood drawn.

Cyrex does not contract with insurance so you will have to pay upfront for the lab. You will submit your payment information to Cyrex when you generate the authorization for the blood draw. Cyrex arrays range from $195 to $580.

Getting your labs done

You will receive a test kit in office or one will be mailed to you. You will be collecting urine and/or saliva samples for these tests. Please watch the test instruction video for more information.

DUTCH Complete: $250

Fill out the payment card with your sample. You will pay the lab directly. You can request CPT codes to submit to your insurance for reimbursement. You will also need the ICD 10 codes from the superbill for this visit to submit to your insurance with the CPT codes and receipt for the labs.

Getting Your Labs Done

All test kits can be completed at home. Tests are urine, stool, or dried blood spot. Many of the kits require avoiding certain foods and supplements for a few days prior to collection. Some kits require multiple days of collection. Please read the test instructions carefully and schedule your sample collection appropriately. 

Payment

I will bill you for the test when I provide you with the kit in office or place the order to have the kit mailed to you. I will bill you the cost of the lab plus a 4% surcharge to cover credit card processing. The lab will bill me the physician price, which is cheaper than patient pay price, when they receive your sample. This way, we can get you a bit of a discount on the labs. If you decide to not do the test after you receive a test kit, please return the test kit to my office and I will refund you.

Spectracell

Getting Your Labs Done

Option 1: Go to Marshall Medical in Cameron Park to get your blood drawn at no additional charge. Bring your requisition form (provided in office), a copy of your insurance card (copied in office), and lab kit. If you are new to the Marshall system, you will have to register first with them. They will take your name and all your information and have you sign a HIPAA form. You may need to be fasting for some of these labs. See the requisition form to check.

Payment

If you have commercial insurance (not Medicare of Medicaid), you will only be responsible for the copay. Spectracell will attempt to bill your insurance but you will not be responsible for the remainder of the cost even if your insurance does not cover it. You will pay with check or credit card when you submit your sample.

If you have Medicare or Medicaid, you will be responsible for paying in full for the test. 

Telomere testing is not covered by insurance at this time.

True Health is out-of-network for most insurances. So you will pay cash pricing. After you get your blood drawn, you will receive a bill in the mail to pay for your tests. 

Ulta Labs

Getting your labs done

At the time of ordering your labs, we will pick the most convenient Quest blood draw site for you. It is recommended that you schedule an appointment for your blood draw to get the best service. Click here to schedule. Bring your test requisition form to your appointment. 

Payment

You pay for your labs at the time of ordering. There is an $8 draw fee for all orders. You will not need to pay anything when you go to Quest for your blood draw.
